Veteran Pathways & Proactive Care is seeking experienced General Practitioners to deliver chronic disease management and structured multidisciplinary case conferencing for veterans and patients with complex, multi-system health needs. The role runs across two structured days each week â an in-clinic day at our Ipswich partner practice and a remote telehealth case-conferencing day â with some flexibility on the days to suit you. This is not a standard locum. You will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans.
