As an RPM 365 Care Coach , you’ll engage patients, track and analyze their data, provide education, and encourage adherence to their care plans. About You You're a licensed LPN/LVN passionate about healthcare innovation and patient outcomes. Your tech-savvy, comfortable with high-volume phone outreach (125+ conversations a week), and thrive in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter. Responsibilities Serve as the primary contact for patients on the RPM365 platform. Assist with device setup and app connectivity. Engage with patients to ensure adherence to care plans.
